United HealthCare for the Empire Plan Basic Medical Program, the Home
Care Advocacy Program (HCAP) and for non-network physical therapy or chiropractic
services. For claim forms, contact United HealthCare at (877) 769-7447,
option 1, or Benefit Services at (607) 255-3936.
ValueOptions for non-network mental health and substance abuse
services. For claim forms, call ValueOptions at (877) 769-7447, option
3.
Caremark for prescriptions filled in 2007 at non-participating
pharmacies or without using your New York Government Employee Benefit
Card. For claim forms, contact Caremark at (877) 769-7447, option 4, or
Benefit Services at (607) 255-3936. Medco is the administrator for prescriptions
filled in 2008.
June 30, 2007 is the last day to submit your 2007 GHI dental claims
to:

Select Benefits Claim Deadline for 2007 Expenses:
You have until March 15, 2008 to incur expenses against any remaining
2007 account balance.
April 30, 2008 is the last day to submit Select Benefits claims for services
incurred during 2007. Mail your claim form and supporting documentation
to: Aon Consulting, Flex Administration #00559-80, PO Box 3002, Arlington
Heights, IL 60006-3002 or use Aon's on-line system to submit
claims.
